talking 0:39 to you about periorbital rejuvenation 0:42 specifically lower lid blepharoplasty with 0:45 fat grafting um this is a topic that i'm 0:48 passionate about because it's something 0:50 that i really enjoy doing for my 0:52 patients and i'm happy to talk about it 0:54 with you all tonight um full disclosure 0:59 before i get into the presentation i 1:01 should mention that this does involve a 1:03 surgical procedure and like any surgery 1:05 there are risks risks include bleeding 1:08 infection bruising swelling asymmetry 1:12 numbness persistent pain i'll talk a 1:15 little bit more about those later but i 1:17 wanted to mention that upfront 1:20 so in terms of background um this 1:22 patient was a beautiful 34 year old 1:25 female who came to me complaining of 1:28 hollowing under her eyes creating the 1:30 appearance of dark circles and she 1:33 wanted to know what could be done to 1:34 improve the under eye area 1:36 um as you can see in this before picture 1:40 she has a very nice smooth upper eyelid 1:42 but when she smiles that's when we really 1:44 appreciate the degree of hollowness that 1:46 she has underneath her eyes so this is 1:48 known as a tear trough deformity the 1:51 nasojugal fold is the fold right here 1:54 and the tear drops are right below there 1:56 um so when we smile we want to fill out 1:58 all the way across from that nasojugal 2:01 fold down to the cheek there should be no 2:04 lines or no hollowness and that's what 2:07 this patient wanted to improve 2:10 um so we talked about her goals and 2:13 expectations and what she wanted to 2:15 achieve with this procedure she wanted a 2:16 more refreshed and youthful appearance 2:19 she wanted to improve the under eye 2:22 circles and she wanted to be able to 2:25 wear less makeup 2:28 so here's her immediately after surgery 2:32 um so it's a little bit swollen and a 2:35 little bit bruised but you can already 2:37 appreciate a fullness underneath her 2:39 eyes and this is a result of injecting 2:41 fat which is shown in this drawing here 2:46 um this drawing demonstrates the tear 2:49 trough and the nasojugal fold and the 2:52 tear drops again and what you see in the 2:55 before is the before picture and the 2:56 after is the after picture 2:58 um the before picture demonstrates the 3:00 hollowness underneath her eyes and the 3:03 after picture demonstrates what fat 3:05 grafting can do to fill in that area 3:08 um the fat is injected in this manner 3:11 very carefully in small amounts and 3:13 placed just below the orbicularis muscle 3:16 which is the muscle that helps you blink 3:18 and then covered by skin and muscle 3:21 um so the fat is not injected right up 3:24 against the skin it's actually injected 3:26 just below the muscle and then covered by 3:29 skin and muscle so this kind of protects 3:31 the fat from being absorbed and also 3:33 from being displaced or migrated 3:36 um it's a very precise technique that 3:39 takes a lot of practice to master 3:42 um so let's talk a little bit more 3:44 about the surgical technique i use 3:46 um i start with creating an incision 3:49 just below the lower eyelash line um 3:52 i use a very sharp blade and i make a 3:54 very small incision just like this i do 3:57 this under local anesthesia with oral 3:59 sedation and most of my patients are 4:01 very comfortable so as soon as i make 4:03 the incision i start placing my 4:05 cannula um the cannula is like a blunt 4:09 tipped tube that is sharp enough to 4:11 penetrate the skin and it's blunt enough 4:13 not to cause any damage to surrounding 4:16 tissue so it's placed just below the 4:19 skin and the incision is made just under 4:21 the eyelash line right here and i use a 4:25 very precise technique to inject the fat 4:28 very slowly very carefully and 4:30 repeatedly so the cannula is inserted 4:33 and then moved around in a fanlike 4:35 fashion to distribute the fat over the 4:38 entire tear trough area 4:41 um this is a picture here of a cannula 4:45 it's a very very fine cannula it's only 4:47 about like a 24 gauge which is a very 4:50 small one and the tip is very blunt but 4:53 it's just sharp enough to penetrate the 4:55 skin and then the fat is injected through 4:58 this cannula um so i inject the fat just 5:01 below the orbicularis muscle and then as 5:04 i'm injecting the fat i'm moving the 5:06 cannula around in this fanlike fashion 5:08 so that the fat is distributed 5:10 precisely over the entire tear trough 5:12 area and 5:13 um once the injections are complete the 5:16 cannula is removed and the incision is 5:18 closed with a suture so i just use one 5:21 or two sutures to close the incision 5:24 very carefully 5:26 um so this is what the incision looks 5:29 like immediately after surgery the incision 5:32 is right here just below the lower 5:34 eyelash line and as you can see it's very 5:37 small um there is a little bit of bruising 5:40 and swelling 5:42 um it will take several weeks for the 5:45 bruising and swelling to completely 5:47 resolve but the incision itself is hidden 5:50 within the natural creases of the lower 5:53 eyelid so it becomes almost invisible 5:55 um so this is a patient of mine who had 5:58 fat grafting with lower lid blepharoplasty 6:01 and you can see she had beautiful results 6:03 she achieved her goal of improving her 6:05 dark circles and looking more refreshed 6:08 and youthful um this is a patient who 6:12 had a little bit of a more severe 6:14 hollowing underneath her eyes and again 6:17 she wanted to improve the hollows and 6:19 dark circles and she was very happy with 6:21 her results 6:22 um one of the things that i really like 6:24 about fat grafting is that it gives a 6:26 very natural result um fat is a very 6:30 biocompatible material your body knows 6:33 how to accept it um so it's not a foreign 6:36 substance that your body is going to 6:38 reject um it's something that your body 6:40 recognizes and knows how to integrate so 6:43 it really gives a very very natural 6:46 result um the other benefit of using fat 6:49 is that it is long lasting um studies 6:52 have
